The transition from children's to adult developmental services can be complicated. Starting early gives families more time to understand the options, complete applications and plan for the support their young person may need. Hopewell works alongside young people and families to navigate the process and connect with adult services.

At 18, the system changes
In Ontario, children’s and adult developmental services are separate systems. As a young person approaches 18, some children’s supports may end and adult services require separate applications. Planning early gives families more time to understand what’s changing, explore their options and apply for the supports.

Your path to adult services

Start at 16: register with DSO
Developmental Services Ontario (DSO) is the front door to all adult developmental services. You can apply as early as age 16, and early registration matters: assessments and funding take time. You'll confirm eligibility and complete an application package with them.

Explore funding
Once confirmed eligible, adults may access Passport funding for community participation and respite. DSO will walk you through what's available, and we're happy to help you understand it too.

Land in adult programs
Hopewell's adult services pick up where Youth Connect leaves off: Hopewell Connect for community, friendships and recreation; Ooster House for overnight respite; and Supportive Living when it's time for a home with 24-hour support.

How Hopewell helps
If your young person is in Youth Connect today, talk to us before they turn 17. We'll help you plan the handoff so there's no gap between youth and adult programs. We can help you understand the DSO application, point you to the right people and introduce the adult programs that fit best.

Where the path leads
Hopewell Connect
Community, friendships, recreation and skill-building for adults. Fee-for-service, so no DSO approval is needed, and Passport funding can often be put toward it.
