The transition from children's to adult developmental services can feel complicated. Starting early creates more time to understand your options, complete applications and plan for the support your young person may need. Hopewell works alongside young people and families through this transition, helping you understand the process and connect with adult services.
Why this matters
Two systems, one transition
In Ontario, supports for children and supports for adults living with a developmental disability run through two different systems. School supports, children's respite and youth programs wind down around a young person's 18th birthday, and adult services are applied for separately. Planning ahead is what gives families the widest set of choices.

From youth services to adult services

Start at 16: register with DSO
Developmental Services Ontario (DSO) is the front door to all adult developmental services. You can apply as early as age 16, and early registration matters: assessments and funding take time. You'll confirm eligibility and complete an application package with them.

Explore funding
Once confirmed eligible, adults may access Passport funding for community participation and respite. DSO will walk you through what's available, and we're happy to help you understand it too.

Land in adult programs
Hopewell's adult services pick up where Youth Connect leaves off: Hopewell Connect for community, friendships and recreation; Ooster House for overnight respite; and Supportive Living when it's time for a home with 24-hour support.

If your young person is in Youth Connect today, talk to us before they turn 17. We'll help you plan the handoff so there's no gap between youth and adult programs. We can help you understand the DSO application, point you to the right people, and introduce the adult programs that fit.

Hopewell Connect
Community, friendships, recreation and skill-building for adults. Fee-for-service, so no DSO approval is needed, and Passport funding can often be put toward it.
