How we collect, use, store and protect your personal and health information, and your rights.
Hopewell Support Services is committed to protecting your privacy and your personal health information (PHI), in keeping with Ontario's Personal Health Information Protection Act (PHIPA). This page explains, in plain language, what we collect, how we use and safeguard it, where it is stored, and the choices and rights you have.
To provide safe, coordinated support, we may collect personal and health information, such as health history, allergies and medical needs, medications, support and behaviour plans, emergency contacts, and consent records. We collect only what we reasonably need to provide your support.
Your information is used by the authorized staff involved in your care, on a need-to-know basis. Sharing within your care team relies on your consent. Sharing with anyone outside your care team requires your explicit consent, and you can withhold or withdraw consent (except where the law requires disclosure, such as a safety concern, subpoena or warrant). We never sell your information or use it for advertising.
We safeguard your information with encryption, secure access controls, regular backups, privacy impact assessments, and ongoing privacy training for our staff and students. Only authorized staff involved in your care can see your information.
Your care records are stored on secure servers located in Canada. Our technology providers process this information on our behalf under contract and do not use it for their own purposes.
We keep your information only as long as needed to provide your support and to meet our legal, regulatory and funding obligations, then dispose of it securely. Secure links we email for signing consents or reviewing plans expire automatically and are deleted.
You may ask to see a copy of your clinical file, ask us to correct your information, ask us to limit who receives it, and withdraw consent. You may also make a privacy complaint, to us, and to Ontario's Information and Privacy Commissioner (IPC) at 1-800-387-0073 or ipc.on.ca.

The section above is about the care records we hold for the people we support. This section is about this website: what happens when you simply visit it, fill in a form, or use one of the services embedded in a page.
You do not need to give us anything to read this website. Like every web server, ours records standard technical information when a page is requested, the page address, your browser type and your IP address, which is used to keep the site running securely and is not used to identify you.
Our message, booking, gift-pledge and feedback forms collect what you type: typically your name, email, phone number and your message. It is sent to Hopewell’s own secure administrative system, where it can be read only by the authorized staff who handle that kind of request. That system’s database is hosted in Canada (Montréal). We use what you send us to answer you and, where relevant, to arrange the service you asked about. We do not sell it, and we do not add you to a mailing list without you asking.
The Feedback & Complaints form has an “I prefer to stay anonymous” option. Choose it and no name, email or phone number is submitted or stored, only what you write. We will not be able to reply to you, but the concern still reaches our team.
This website does not use advertising cookies and does not track you across other websites. We use Google Analytics 4 to understand how the site is used in aggregate, which pages are visited, roughly where visitors come from, and whether people find what they need. It sets a cookie in your browser and shares this usage data with Google. It tells us nothing about who you are, and we never connect it to a care record. You can opt out for every website using Google’s browser add-on, or by blocking cookies in your browser settings, the site works normally either way.
Some parts of this site are provided by trusted third parties. When you use one of these, information goes to that company under its own privacy policy, and some of them store or process data outside Canada:

Every identifiable person shown on this website appears with their consent, or their substitute decision-maker’s consent where applicable. Consent can be withdrawn at any time, contact our Privacy Contact above and we will remove the image.
